We are the sole owners of the information collected on this site. That information may include personally identifiable information such as your name, telephone number, and email address, and it may include technical information such as your IP address, browser type, device type, access times, and referring website. We only collect information that you give us voluntarily, such as when you complete the appointment request form or contact us by phone or email. We use your information to respond to you about the reason you contacted us. We do not sell or rent your information, and we do not disclose it to outside parties except as necessary to fulfill your request or as required by law.
The appointment request form
When you submit the appointment request form, the details you enter (your name, phone number, email address, preferred office, the reason for your visit, and any message you add) are delivered by email to our practice so a team member can follow up. The form is deliberately not intended for medical details, test results, or any description of a health condition, and we ask you not to include them. If you need to share clinical information, please call the office so we can collect it through a channel appropriate for protected health information.
Cookies and website analytics
This site uses cookies and similar technologies to understand how visitors find and use our pages so we can improve them. We use Google Analytics, deployed through Google Tag Manager, which sets cookies and processes information such as pages viewed, approximate location, device and browser type, and how you arrived at the site. This measurement data is used in aggregate to improve the website; it is not used to identify you personally. You can block or delete cookies in your browser settings, and you may opt out of Google Analytics using Google's browser add-on. Pages that embed video use YouTube's privacy-enhanced mode.
Third-party services
Some features are provided by third parties who handle your information under their own privacy policies. Online appointment scheduling is provided by Zocdoc; when you book through it you are interacting with Zocdoc's platform. Videos are embedded from YouTube, and maps and directions link to Google Maps. We encourage you to review the privacy policy of any third-party service you choose to use.
Your access to and control over your information
You may opt out of future contact from us at any time. By contacting us at the phone number or address on this site you can, at any time: see what data we hold about you, if any; correct any data we hold about you; ask us to delete any data we hold about you; unsubscribe from future emails; and raise any concern about our use of your data. California residents may have additional rights regarding personal information under California law, and we honor those requests through the same contact channels.
Security
We take precautions to protect your information both online and offline. Information submitted through this site is transmitted over an encrypted connection. Offline, only employees who need the information to perform a specific job are granted access to personally identifiable information, and the systems on which that information is stored are kept in a secure environment.

In accordance with CalOPPA: we do not sell personally identifiable information to third parties; we do not permit third parties to collect personally identifiable information about visitors from this website for their own independent purposes; and we disclose above the analytics technologies this site uses, so you can make an informed choice about cookies through your browser settings.
Children under thirteen
We do not knowingly collect personally identifiable information from children under the age of thirteen through this website. If you are under thirteen, please ask a parent or guardian for permission before using this website. Children are of course welcome as patients of the practice; this provision concerns information collected through the website.
